Job summary

PAM Health is seeking a Patient Assessment Coordinator to ensure timely data collection for Medicare patients and to support CMS reporting requirements. You will coordinate IRF/PAI data, review forms for completeness, and educate staff on CMS guidelines.

Qualifications

• Graduate of an accredited OT, PT, SLP, or RN program with current licensure in the state.
• ASN required; Bachelor’s degree preferred for nursing.
• Master’s degree required for OT/PT/SLP.
• Current Basic Life Support (BLS)/CPR card.
• Three (3) to five (5) years of experience in a patient assessment coordinator role may be substituted for degree and license.
• Three (3) to five (5) years of experience in the care of patients in an acute hospital, rehabilitation, or LTACH setting.

Responsibilities

• Coordinate the process of providing accurate and timely data to the appropriate Medicare entities (PFS and quality reporting) for IRF/PAI forms.
• Oversee all aspects of data collection on the IRF/PAI and ensure supporting documentation.
• Review each IRF/PAI form completely, accurately, and timely.
• Monitor the accuracy of FIM scoring and patient care needs in the eRehabData system.
• Coordinate collection of patient information from medical records and complete assigned IRF/PAI sections.
• Set completion dates for the multi-disciplinary team (MDT); develop and interpret policies and procedures.
• Serve as the primary resource for Medicare IRF PFS criteria and quality reporting requirements.
