Job summary

PAM Health in San Antonio, TX, seeks a Patient Assessment Coordinator to manage IRF/PAI data collection, ensure CMS-compliant documentation, and support scoring accuracy in the eRehabData system.

You will coordinate information from patient records, set completion dates for the multi-disciplinary team, and serve as the primary resource for problem-solving related to Medicare’s IRF/PAI processes and quality reporting requirements.

Qualifications

• Graduate of an accredited OT, PT, SLP or RN program with current state licensure.
• ASN required; Bachelor’s degree preferred for nursing.
• Master’s degree required for OT/PT/SLP. BLS/CPR current.

Responsibilities

• Coordinate data collection for IRF/PAI and CMS guidelines.
• Oversee IRF/PAI scoring accuracy and documentation.
• Input information into the eRehabData system from patient records.
• Set IRF/PAI completion dates for the multi-disciplinary team.
• Develop policies and serve as a primary problem-solving resource for Medicare IRF processes.
