Job summary

PAM Health Rehabilitation Hospital of Round Rock is seeking a Patient Assessment Coordinator to manage data collection for the Medicare Prospective Payment System and quality reporting. You will ensure IRF/PAI completion, accurate scoring of functional status, and timely submission into the eRehabData system.

Responsibilities include coordinating patient information from medical records, setting IRF/PAI completion dates for the multi-disciplinary team, and serving as the primary resource for CMS

Qualifications

• Licensure/credentials: current state license; RN/OT/PT/SLP with appropriate credentials.
• ASN required; Bachelor’s degree preferred for nursing; Master’s degree required for OT/PT/SLP.

Responsibilities

• Coordinate data collection for Medicare PPS and quality reporting.
• Oversee IRF/PAI data accuracy and documentation.
