Job summary

PAM Health Rehabilitation Hospital of Katy is seeking a Patient Assessment Coordinator to manage data collection for IRF/PAI, ensure CMS guideline compliance, and educate staff on requirements. This role involves reviewing forms for accuracy, coordinating information from patient records, and supporting the multi-disciplinary team in timely completion.

Ideal candidates will have 3–5 years’ experience in a similar coordination role, current BLS/CPR, and licensure in an OT/PT/SLP/RN pathway with

Qualifications

• Graduate of an accredited OT, PT, SLP, or RN program with current state licensure.
• Current Basic Life Support (BLS)/CPR card.
• Three to five years of experience in a patient assessment coordinator role in an acute hospital, rehabilitation, or LTACH setting.

Responsibilities

• Coordinate data collection for the IRF/PAI and CMS guidelines.
• Oversee scanning and accuracy of IRF/PAI forms and ensure supporting documentation is present.
• Input IRF/PAI information into the eRehabData system and monitor scoring accuracy.
• Set IRF/PAI completion dates for the multi-disciplinary team and maintain timelines.
• Develop and interpret policies and procedures; serve as primary resource for Medicare PPS requirements.

Education

ASN required

Bachelor’s degree preferred for nursing

Master’s degree required for OT/PT/SLP
