AndHealth LLC seeks a Patient & Provider Success Liaison to drive enrollment and seamless patient access within partner practices. You will identify eligible patients, facilitate onboarding in the practice setting, and work directly in EHRs to ensure fast, brand-consistent enrollment.

You will also build relationships with practice staff, deliver white-glove referral support, and train teams on workflows while traveling to partner sites several days per week.

Formación

• Bachelor's Degree required.
• 5+ years in a field reimbursement, patient access, or clinical operations role.
• Willingness to travel regularly; on-site at partner practices most days.
• Experience with specialty patient populations is a plus.
• Familiarity with AI tools.

Responsabilidades

• Identify eligible patients and enroll them within partner practices and EHRs.
• Queue patients for provider review and manage enrollment transitions.
• Support referrals with white-glove service and proactive communication.
• Train practice staff on referral workflows and maintain documentation.
• Track enrollment funnel and surface process gaps with remedies.
