Job summary

Jobtailor is seeking a healthcare navigator to deliver exceptional patient-centered support in Boise, Idaho. The role acts as the primary contact for patients and families, helps resolve barriers to care, and coordinates ambulatory referrals within the Epic environment.

Responsibilities include safeguarding patient confidentiality, scheduling within the EMR, collecting demographic data, and maintaining smooth communication with clinic staff.

Qualifications

• Excellent verbal and written communication skills in English.
• High School Diploma or equivalent required.
• Proficient in Epic, Outlook, MS Office, and practice management systems.
• Working knowledge of medical terminology and healthcare systems, including insurance, Medicare, and Medicaid, preferred.
• Ability to learn and maintain comprehensive knowledge of the healthcare community, including specialties and providers.

Responsibilities

• Deliver exceptional customer service to patients, physicians, and staff
• Act as a system navigator and primary contact for patients and families
• Address patient and family questions and concerns
• Assist patients in overcoming healthcare, financial, or social barriers
• Arrange interpreters and provide transportation resources
• Facilitate ordering and processing of ambulatory referrals
• Route calls to clinical staff when trigger words or urgent concerns are identified
• Maintain patient confidentiality in compliance with HIPAA standards
• Schedule appointments in the electronic health record using provider-specific templates and preferences
• Collect and validate patient demographic and insurance information
• Collaborate with clinic contacts on unusual requests and new patient appointments
• Handle multiple phone lines professionally
• Solve problems, improve workflows, and maintain a positive work environment independently and collaboratively
