Job summary

AdventHealth in Winter Haven, Florida is seeking a full-time Patient Services/Care Coordinator to triage calls, coordinate appointments, and educate patients about their plans of care. This role supports patients through the entire care journey and collaborates with facilities, physicians, and home health providers.

Minimum high school diploma required; 1+ medical office experience preferred. Day shift in a full-time schedule with benefits from day one and a competitive hourly pay range.

Qualifications

• Basic knowledge of Microsoft Office products.
• Customer service and service recovery.
• Basic insurance knowledge.

Responsibilities

• Triages incoming calls from facilities, patients, physician offices, hospitals, pharmacies, and home health.
• Coordinates new patient appointments and disseminates information based on acuity level.
• Educates and screens all new patients in preparation for their appointments to ensure their plan of care is appropriate.
• Navigates patients through their entire plan of care.
• Other duties as assigned.
