Job summary

Enhabit Home Health & Hospice is seeking a dedicated Scheduler/Care Coordinator in Florida to coordinate patient scheduling with branch clinicians and ensure seamless care delivery. The role involves clear communication with field staff, physicians, and families to maintain continuity of care across teams.

The ideal candidate has a high school diploma, in-state LPN/LVN licensure or relevant home health experience, and basic tech skills including mobile device usage.

Qualifications

• High school diploma or equivalent required.
• Must be licensed in-state as LPN/LVN or have 1 year of home health/hospice/pediatrics experience if not licensed.
• Basic technology skills including use of a mobile device and scheduling software.

Responsibilities

• Schedule patients to branch field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other providers to ensure continuity of care.
• Manage the on-call notebook and hospitalization logs to enhance stakeholder communication.
