Job summary

Lincolnshire Community Health Services NHS Trust is seeking a motivated Care and Wellbeing Navigator to support the Tier 3 Weight Management Service. The role acts as a central point of coordination for referrals, patient engagement and service administration, ensuring a seamless patient journey.

You will work closely with clinicians and partner organisations, contributing to data management, meeting coordination and performance monitoring, while maintaining compassionate, person-centred care.

Qualifications

• Experience in an administrative, coordination or patient-facing role.
• Highly organised and able to manage competing priorities.
• Communicates confidently and professionally with a wide range of people.
• Strong IT skills, including Microsoft Office and database systems.
• Can work independently while contributing positively to a wider team.
• Demonstrates a compassionate, person-centred approach.
• Is interested in supporting health improvement and behaviour change.
• Experience within healthcare, primary care, community services or referral management pathways would be advantageous but is not essential.
