Job summary

CHC Addiction Services in Akron, OH seeks a Patient Care Liaison to enhance patient access and support clinical workflows. You will coordinate appointments, verify demographics and insurance, and facilitate referrals across the care team.

The role combines administrative, patient service, and clinical support tasks to ensure a patient-centered experience and accurate documentation, while maintaining confidentiality in a fast-paced healthcare setting.

Qualifications

• Minimum of 2 years’ experience in medical office operations, front desk registration, clinical support, or medical assisting.
• High school diploma or equivalent required. Medical Assistant certification preferred.
• Experience using electronic medical record systems.
• Strong organizational and multitasking abilities with exceptional attention to detail.
• Strong communication and customer service skills. Ability to work independently and collaboratively.
• Understanding of patient confidentiality standards and HIPAA.
• Ability to function effectively in a fast-paced, highly regulated healthcare environment.

Responsibilities

• Welcome and assist patients in a professional, compassionate, and confidential manner.
• Schedule, confirm, and coordinate patient appointments.
• Verify demographic, insurance, and registration information.
• Coordinate referrals to specialty providers and community resources.
• Track referral status and obtain consultation reports.
• Prepare patient charts and ensure documentation is complete prior to appointments.
• Support providers with administrative and clinical workflow tasks.
• Ensure patient information is documented accurately and timely.
• Maintain confidentiality and comply with HIPAA regulations.
• Serve as a liaison between patients, providers, behavioral health staff, nursing, care management, and external healthcare organizations.
• Participate in team meetings and collaborate to improve patient outcomes.
• Support clinic operations by assisting with special projects and other duties as assigned.
