Qualifications

• At least 3 years of case management experience.
• Experience working with justice-involved individuals.
• Lived experience as a formerly incarcerated or justice-involved individual desired.
• Strong written and verbal communication and public speaking skills.
• Ability to build collaborations and partnerships with businesses and stakeholders.
• Ability to pass a background check for facility access.

Responsibilities

• Plan and provide intensive, face-to-face case management with weekly participant meetings.
• Assess participants' needs and develop Individual Career Plans.
• Refer participants to housing, mental and physical health services and other providers.
• Coordinate with Siouxland Pathway Employer Specialist to connect to employers and training partners.
• Prepare participants for employment and assist with training enrollment.
• Maintain accurate records and input data in the system.
• Contribute to quarterly reports per DOL REO grant requirements.
• Represent the program in outreach and stakeholder presentations.
