Job summary

Siouxland Human Investment Partnership is seeking a dedicated Case Manager to support participants across Woodbury County, IA. This role provides intensive, face-to-face services, helps participants set achievable goals, and connects them with training, housing, and health resources.

You will work with partners like Iowa Workforce Development to promote employment readiness and ensure accurate record-keeping and reporting for program requirements.

Qualifications

• BA or BS in Social Work or closely related discipline preferred or equivalency.
• At least 3 years of case management experience.
• Experience with justice-involved individuals (adults or youth) desirable.
• Lived experience as formerly incarcerated or justice-involved encouraged.
• Good written and verbal skills; strong public speaking ability.
• Ability to organize work with limited supervision and build collaborations.
• Must pass a background check to enter correctional facilities.

Responsibilities

• Provide intensive, face-to-face case management to enrolled participants, weekly at enrollment.
• Assess participant needs and assist in creating Individual Career Plans (ICP).
• Refer participants to housing, health services, and other partners as needed.
• Coordinate outreach materials and recruit employers for program opportunities.
• Prepare participants for employment and monitor progress in ICP execution.
• Contribute to quarterly reports for DOL REO grant requirements.
