Job summary

Siouxland Human Investment Partnership seeks a dedicated Case Manager to support participants in Woodbury County, Iowa, guiding them through enrollment, goal setting, and connections with training providers and potential employers.

You will provide intensive, weekly, face-to-face case management, assess needs, help participants create an Individual Career Plan (ICP), and coordinate referrals to housing, health, and employment services.

Qualifications

• BA/BS in Social Work or related field preferred.
• At least 3 years of case management experience.
• Experience working with justice-involved individuals preferred.
• Strong written and verbal communication skills.

Responsibilities

• Provide intensive, face-to-face case management to enrolled participants; meet weekly at enrollment.
• Assess Siouxland Pathway participants to determine needs and employability.
• Assist participants in setting realistic goals via an Individual Career Plan (ICP).
• Refer participants to partners for housing, health and employment services.
• Prepare participants for employment and maintain accurate records and reports.
