Qualifikationen

• Bachelor's degree preferred in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or related field.
• Experience with mental health, substance use, justice-involved populations, or community-based services preferred.
• Ability to provide own laptop or iPad for charting and documentation.

Aufgaben

• Provide Case Management and/or CBRS services to adults with mental health, behavioral health, and reintegration needs.
• Complete assessments and assist clients in identifying strengths, barriers, goals, and support needs.
• Develop individualized service plans in collaboration with clients and their support systems.
• Assist clients with accessing housing, employment opportunities, educational resources, healthcare, transportation, and community services.
• Teach and reinforce independent living skills, coping, problem-solving, communication, and community integration strategies.
• Advocate for clients and assist them in navigating complex systems and community resources.
• Collaborate with probation and parole officers, treatment providers, employers, housing agencies, and other community partners.
• Maintain accurate, timely, and compliant documentation in accordance with Medicaid and agency standards.
