Porter-Starke Main Office seeks a PATH Case Manager to provide outreach to homeless individuals and those at risk, connecting them with housing, health, and social services. You will collaborate with community partners and document interventions according to department standards.

The role involves driving clients as needed, assisting with SOAR outreach, and data entry into HMIS, with a focus on advocacy and access to entitlements like Medicaid and SNAP.

Qualifications

• Bachelor’s Degree required.
• Experience in mental health or outreach is a plus.
• Experience with vulnerable populations is a plus.

Responsibilities

• Provides street outreach.
• Works with community partners to provide services to homeless individuals.
• Documents interventions per department standards.
• Drives and transports clients for PATH-related duties.
• Data entry into HMIS.
• Assists with SSI/SSDI SOAR outreach.
• Other duties as assigned.
