Job summary

StarCare Specialty Health System in Lubbock, TX seeks a full-time PATH Service Coordinator to support individuals experiencing homelessness or at risk of homelessness with serious mental illness and/or co-occurring substance use disorders. You will conduct outreach, assessments, and care coordination to connect clients with housing, health care, and recovery services.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or related field required.
• Minimum of one year of experience in case management, outreach, behavioral health, homeless services, or related support services preferred.
• Experience with homelessness, serious mental illness, substance use disorders, or co-occurring disorders preferred.
• Knowledge of community resources, housing programs, behavioral health systems, and recovery-oriented practices preferred.
• Ability to build rapport and engage individuals from diverse backgrounds.

Responsibilities

• Conduct active outreach to unhoused individuals by meeting them where they are (encampments, shelters, etc.).
• Distribute program materials and build relationships with shelters and service providers to improve access to care.
• Develop individualized plans reflecting client goals and review progress regularly.
• Help clients access housing, mental health care, and substance use treatment.
• Connect clients with primary care, job training, education, income support, and other services.
• Complete required documentation and assessments to track progress and continuity of care.
• Collaborate with multidisciplinary teams to ensure coordinated care.
• Use trauma-informed, person-centered, recovery-oriented practices in engagement.
• Ensure services comply with policies and regulatory requirements.
• Maintain regular attendance and keep credentials current.
• Complete other department projects as assigned.
