Job summary

Legacy Treatment Services is seeking an Entry Level Case Manager to join PATH in Hainesport, NJ. The role provides outreach case management to adults dealing with serious mental illness who are homeless or at risk of homelessness.

Responsibilities include coordinating with referral sources, assessing consumer needs, and ensuring appropriate age-related care while operating an Agency vehicle as required. This is a full-time benefited position with comprehensive benefits.

Qualifications

• Must have case management experience.
• Bachelor’s degree or higher in a social science field is required.
• Eligible to drive agency vehicle with acceptable driving record.

Responsibilities

• Provide outreach case management services to consumers.
• Maintain contact with referring agencies, providers, families, and homeless individuals.
• Assess data to identify needs and provide appropriate care.
