Job summary

Maimonides Medical Center seeks a Manager of Partner Engagement and Performance Improvement to lead engagement with provider partners and drive value-based care initiatives. You will monitor performance, coach partners, and implement population health strategies to improve care quality and outcomes.

The role includes onboarding new partners, developing training, and coordinating cross-team efforts. Strong data analysis and relationship-building skills are essential for success in this leadership

Qualifications

• Master’s degree in public health, public administration, health services administration, business, or related fields
• Minimum 4 years of experience in healthcare with quality improvement and ambulatory care preferred
• Minimum 2 years of project management experience
• Experience implementing change efforts in healthcare organizations
• Experience with health IT/EMR systems preferred
• Excellent interpersonal, problem-solving, and communication skills

Responsibilities

• Cultivate relationships with partner organizations and serve as primary contact
• Collaborate with internal teams to tailor engagement strategies
• Attend partner site meetings and follow up on action items
• Monitor partner engagement and develop remediation plans when needed
• Monitor and analyze performance data for partners and share actionable insights
• Develop communication strategies to share performance data with partners
• Support data retrieval and reporting from partner sites
• Lead onboarding for new partners and coach on value-based care
• Develop and deliver training to enhance value-based care capabilities
• Identify resources and connect practices with tools and support services
• Design and implement performance improvement strategies with partners
• Provide ongoing coaching to sustain improvement efforts
• Develop reports, presentations and training collateral
• Supervise program associates and coordinators
