Job summary

Applewood Centers, a private, nonprofit agency, seeks a Behavioral Health Respite Provider to support youth ages 8-17 enrolled in the OhioRise program. Sessions run ~3 hours, in-home or community-based, with evenings and weekends as needed. Supervises youth using evidence-based strategies to foster social, emotional, and behavioral development.

Responsibilities include transportation, activity planning, collaboration with the care team, and maintaining confidentiality and quality standards.

Qualifications

• High School diploma required; 3+ years in social services preferred.
• QMHS certification must be obtained before billing services.
• Experience with youth with mental health/behavioral challenges preferred.
• Must have valid Ohio driver’s license, access to a vehicle and insurance.

Responsibilities

• Provide short-term direct care to youth in OhioRise program.
• Communicate with youth, families, and treatment team.
• Update family on youth’s activities and progress.
• Provide transportation to activities.
• Plan age-appropriate recreational, educational and therapeutic activities.
• Maintain safe, structured environment per care plan.
• Collaborate with treatment team for continuity of care.
• Maintain documentation per licensing standards.
