Job summary

CareMed Health Solutions is seeking a Wound NP to provide wound care services to residents in long-term care facilities. The role requires an active Kentucky NP license, board certification, and optional DEA licensure.

You will work with an interdisciplinary team to manage wounds and coordinate care with primary physicians. This part-time position offers opportunities to specialize in chronic wounds, venous/arterial ulcers, and pressure injuries, with benefits including education support, PT/FT

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board certification in a relevant field.
• DEA licensure may be optional.
• Clinical background in adult, family, or geriatric care.
• Wound care experience with chronic or infected wounds.
• Ability to diagnose and treat diabetic, arterial, venous, or mixed wounds.
• Staging pressure injuries per NPIAP guidelines.
• Familiarity with wound care products and DME like wound VACs.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per Kentucky law.
• Order and review treatments and diagnostics; follow up as needed.
• Communicate changes in patient condition with nursing home staff.

While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.

• Prescribe appropriate medications, including controlled substances in accordance with KY Law.

• Order, review, and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.

• Board Certification.

• DEA licensure (Optional)

• Clinical background in adult, family, or geriatric specialties.

• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

• Must be comfortable staging pressure injuries according to the NPIAP

• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

• Strong preference to having wound certification or be open to getting certified

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to work within an interdisciplinary care team to coordinate patient care.
