Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to serve residents in skilled nursing facilities in Washington. You will diagnose, treat, and manage wounds, prescribe medications in line with WA law, and coordinate with facility staff and the primary care physician.

The role emphasizes collaboration within an interdisciplinary team, experience with wound VACs and advanced wound care products, and comfort with EMR systems in a long-term care setting.

Qualifications

• Current, unrestricted NP license in WA
• Board Certification
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties
• Clinical background of wound care and related wound types
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiologies wounds or Kennedy Ulcers
• Staging pressure injuries per the NPIAP
• Familiar with wound care products; collagens, alginates, foams, silvers, etc.
• Experience with wound vacs and other DME
• Wound certification desirable or open to getting certified
• Experience working in a long-term care setting
• Ability to work with an EMR
• Ability to work within an interdisciplinary care team to coordinate patient care

Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with Washington Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with the primary care physician.
