Infinite Medical P.C. seeks a licensed Primary Care NP/PA to provide on-site care in long-term care facilities around Harwich, MA.

This is a part-time weekend position designed for providers who value meaningful patient relationships and collaborative practice. The role involves exams, diagnoses, treatment planning, medication management, and coordination with facility staff to deliver elder‑care that emphasizes comfort, safety, and individualized goals.

Formación

• Active and unrestricted NP/PA/Physician license.
• Massachusetts license in good standing.
• Minimum 6 months experience in long‑term care or skilled nursing facilities.

Responsabilidades

• Provide primary care services to residents in skilled nursing and long‑term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
