Job summary

ALC Health Care Services seeks an experienced hospice RN to plan, organize and direct hospice care, emphasizing community health education and home-based services in Oak Forest, Illinois.

The RN conducts comprehensive assessments, develops and revises plans of care, administers prescribed treatments, counsels patients and families, and collaborates with the hospice team to address physical and emotional needs at the patient's home.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years in acute care and hospice/home care experience.
• Current licensure in the state and CPR certification.
• Bachelor’s degree preferred.

Responsibilities

• Completes initial and ongoing patient and family assessments to determine hospice needs.
• Provides comprehensive nursing care following the nursing process.
• Develops and revises the plan of care with patient and family involvement.
• Regularly re-evaluates patient and family needs and revises the plan as necessary.
• Administers medications and treatments per physician’s orders.
• Counsels patient and family and provides health care instructions.
• Assists with activities of daily living and supports home-based care.
