Job summary

Hackensack Meridian Health is seeking an Advanced Practice Nurse for the Transition of Care Unit. This part-time position covers 20 hours per week, including every Saturday and Monday.

The role emphasizes leadership, direct clinical practice, collaboration, and patient education within a coordinated care model. The APN provides comprehensive assessments, orders diagnostics, prescribes medications, and participates in interdisciplinary rounds to ensure safe transitions of care.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting and NP practice.
• NPI and prescriptive authority per NJ regulations.

Responsibilities

• Performs and documents accurate history and physical assessments appropriate for age and clinical problem.
• Orders and interprets labs and diagnostics in collaboration with physicians.
• Manages general medical/surgical conditions with knowledge of etiology, pathophysiology, and pharmacology.
• Prescribes medications safely in accordance with best practices and hospital policy.
• Promotes APN practice recognition through conferences, publications, and presentations.
• Provides patient- and family-centered care and education.
• Adheres to scope of practice and hospital protocols.
• Participates in clinical education and cross-disciplinary rounds.
