Job summary

Colfax Health and Rehabilitation of Cascadia is seeking a compassionate Part-Time Speech Language Pathologist to support our geriatric rehabilitation program. You will evaluate, treat, and monitor residents to improve communication, cognition, and swallowing.

Join a collaborative team in a sub-acute and skilled nursing setting, working 2–3 days per week with flexible scheduling and opportunities to impact quality of life through evidence-based therapy and patient-centered care.

Qualifications

• Bachelor of Science degree in Speech Language Pathology required.
• Current license in Speech Language Therapy as required by state law.
• Valid CPR certification required.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Screen, assess, and evaluate residents/patients with speech, language, cognitive-communication, voice, swallowing, and fluency disorders.
• Develop individualized care plans based on patient needs, physician directives, and rehabilitation goals.
• Provide direct speech therapy services to address communication, cognitive, and swallowing impairments.
• Plan, direct, and conduct rehabilitative treatment programs to improve functional outcomes and communicative effectiveness.
• Administer, score, and interpret specialized speech and hearing assessments.
• Create customized treatment tools and interventions tailored to individual patient needs.
• Develop treatment plans with measurable short- and long-term goals.
• Maintain accurate, timely, and thorough clinical documentation, including evaluations, daily treatment notes, progress reports, and weekly summaries.
• Educate patients, families, and caregivers on therapy techniques, treatment goals, and strategies for practicing skills outside of therapy sessions.
• Communicate speech therapy programs and progress updates to family members and guardians.
• Work closely with physicians, nurses, rehabilitation therapists, and interdisciplinary care teams to coordinate patient-centered care.
• Deliver high-quality therapy services within a sub-acute and skilled nursing facility setting.
• Ensure punctuality, professionalism, and dependable attendance for assigned shifts.
• Maintain resident and patient privacy in accordance with HIPAA regulations.
