Job summary

IU Health Fort Wayne Hospital seeks a Social Worker (MSW/LSW) for part-time, rotating weekends. The social worker provides direct support to patients and families, partners with the care team, and leads discharge planning to ensure safe, timely care.

Responsibilities include crisis intervention, counseling, abuse/neglect case liaison with protective services, and coordination with guardianship, foster care, adoption, and end-of-life services.

Qualifications

• Master's Degree in Social Work is required.
• Permanent LSW licensure is required.
• Certification in Crisis Prevention Intervention or an alternative de-escalation training is required within 6 months of hire.
• Knowledge of evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention.
• Knowledge of disease process and health-related social needs.
• Complex discharge planning skills to ensure safe transitions.
• Knowledge of state and county protective legal mandates.
• Ability to coordinate goals-of-care conversations with diverse populations.
• Experience collaborating with hospital staff and community agencies.

Responsibilities

• Meets face-to-face with patients and families to assess needs and coordinate care.
• Communicates with the care team to ensure timely services and education.
• Interprets complex needs and models caring practices for others.
• Assesses psychosocial risk factors and coping abilities.
• Counsels patients and families on emotional, social, and financial implications.
• Mobilizes family and community resources to meet needs.
• Provides mental health assessments upon physician referral.
• Performs crisis intervention to facilitate discharge planning.
• Serves as liaison with protective services and guardianship/adoption processes.
• Provides specialized mental health services including crisis intervention and referrals.
