Job summary

Responsibilities

  • Develops social history and assessment of resident, including problems, strengths, needs and care plan implications.
  • Assists with the coordination of Health Center admissions and assessments.
  • Assists residents and families with transition, orientation to facility, services and rights.
  • Participates in development of the individual’s care plan including goals and approaches for Social Services.
  • Attends scheduled care planning conferences, addresses issues, encourages resident and family participation.
  • Ensures Advanced Directives are complete and in resident charts.
  • Completes all mandatory documentation in a timely fashion as established by regulations.
  • Maintains confidentiality of admission information and resident records per HIPAA and Code of Ethics.
  • Maintains regular relationships with family to discuss needs, mediates issues between resident/family/staff.
  • Coordinates discharges/transfers to ensure continuity of care and develops a discharge plan with residents/families and staff.
  • Provides/identifies referrals, agency and/or community resources for resident/family needs.
  • Coordinates Support Groups, participates in policy decisions affecting resident life and advocates for Residents Rights.

Salary is $20/hr

  • Develops a comprehensive social history and assessment of the resident that includes problems and strengths, special needs and preferences (social, religious and/or cultural) and the implications for the care plan.
  • Assists with the coordination of Health Center admissions and assessments.
  • Assists the resident/family with the transition to the new environment, orients the resident/family to the facility, services, limitations and resident’s rights.
  • Participates in the development of the individual’s care plan including the goals and approaches for the Social Services component.
  • Attends scheduled care planning conferences, addressing particular issues of schedule, treatment and environment, and encourages residents and family (as appropriate) to participate in care planning conferences.
  • Ensures that any Advanced Directives are complete and maintained and placed in resident charts.
  • Completes all mandatory documentation in a timely (determined) fashion as established by applicable regulations.
  • Maintains confidentiality of all admission information and resident records following HIPPA and the community’s Code of Ethics.
  • Maintains regular and on-going relationship with family to discuss needs or concerns, mediates issues that may arise between resident/family/staff.
  • Coordinates discharges/transfers to ensure the appropriateness and continuity of care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
