Job summary

All Jobs seeks a qualified social services coordinator to support residents in a Jacksonville, FL facility. You will develop social histories, coordinate admissions, and guide residents and families through transitions.

The role requires at least 1 year of supervised healthcare experience, excellent communication, and the ability to manage care planning documentation in a regulated environment. Salary is $20/hour and a high school diploma is required.

Qualifications

  • At least 1 year of supervised healthcare experience with elderly residents and families.
  • Excellent oral, written and interpersonal skills; professional demeanor under stress.

Responsibilities

  • Develop a comprehensive social history and assessment for the care plan.
  • Assist with coordination of admissions and assessments.
  • Assist residents and families with transitions and orientation to the facility and rights.
  • Participate in developing the individual care plan with goals and approaches for Social Services.
  • Attend care planning conferences and encourage participation by residents and families.
  • Ensure Advanced Directives are complete and in resident charts.
  • Complete mandatory documentation timely per regulations.
  • Maintain confidentiality of admission information per HIPAA and ethics policy.
  • Maintain ongoing relationships with family to discuss needs or concerns and mediate issues.
  • Coordinate discharges/transfers and develop comprehensive discharge plans.
  • Provide referrals to community resources and coordinate support groups.
  • Advocate for Residents' Rights.

Skilled experience preferred.

Salary is $20/hr

  • Develops a comprehensive social history and assessment of the resident that includes problems and strengths, special needs and preferences (social, religious and/or cultural) and the implications for the care plan.
  • Assists with the coordination of Health Center admissions and assessments.
  • Assists the resident/family with the transition to the new environment, orients the resident/family to the facility, services, limitations and resident’s rights.
  • Participates in the development of the individual’s care plan including the goals and approaches for the Social Services component.
  • Attends scheduled care planning conferences, addressing particular issues of schedule, treatment and environment, and encourages residents and family (as appropriate) to participate in care planning conferences.
  • Ensures that any Advanced Directives are complete and maintained and placed in resident charts.
  • Completes all mandatory documentation in a timely (determined) fashion as established by applicable regulations.
  • Maintains confidentiality of all admission information and resident records following HIPPA and the community’s Code of Ethics.
  • Maintains regular and on-going relationship with family to discuss needs or concerns, mediates issues that may arise between resident/family/staff.
  • Coordinates discharges/transfers to ensure the appropriateness and continuity of care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
  • Provide/identify referral, agency and/or community resources to meet the needs of the resident/family.
  • Coordinate Support Groups, participates in policy decisions (as they affect resident life issues) and advocates for Residents Rights.
QUALIFICATIONS:
  • At least 1 year’s experience of supervised work, in a health care setting working directly with individuals, residents and families of the elderly.
  • Excellent oral, written and interpersonal skills. Must present a professional manner and be able to deal with situations that may be stressful.
