Job summary

Qualifications

Responsibilities

• Meet with tenants to assess needs and eligibility for services.
• Coordinate referrals to community-based services and entitlement programs.
• Monitor ongoing services and maintain confidential records.
• Collaborate with Social Services Manager and Property Manager on service delivery.
• Organize on-site programming and liaise with community providers.
• Support residents in developing informal supports networks.
