Job summary

Jewish Association Serving the Aging (JASA) in Brooklyn seeks a compassionate Social Worker for the Brooklyn Case Management Program. You will assess, plan, and coordinate services for older adults, helping them stay independent and connected in their homes and communities.

You will conduct home visits, develop tailored plans, provide counseling, and connect clients with benefits and resources. Strong communication and organizational skills are essential.

Qualifications

• MSW or equivalent master’s degree required.
• LMSW credential preferred; NY certification preferred where applicable.

Responsibilities

• Conduct comprehensive assessments of clients in home or office locations using NYC Aging data system.
• Develop and maintain written case plans tailored to each client’s needs and goals.
• Provide counseling, guidance, and education to clients and families.
• Screen clients for eligibility for benefits, entitlements and other programs.
• Reassess client needs and monitor services to ensure appropriate support.
• Maintain accurate client records and case documentation.
• Coordinate referrals and services including transportation, home care, meals on wheels, and public assistance.
• Advocate for clients to access JASA and NYC Aging relief funds; connect to community resources.
• Prepare reports, contribute to performance metrics, and supervise staff as assigned.
