Job summary

InterCare Community Health Network in Otsego, MI invites applications for a Part-time Community Health Worker (CHW) in the School Based program. Primary hours are Monday, Tuesday, and Thursday from 8:00 a.m. to 5:00 p.m., with some evenings as needed.

Compensation starts at $19.52 per hour with a benefits package including PTO, sick time, and 10 paid holidays. The CHW will navigate community services, enroll patients in programs, and work closely with the Student Health Center team to ensure

Qualifications

• Fluent in Spanish and English (written and spoken).
• High school diploma or equivalent.
• 126 hours CHW Michigan-specific training, 4 hours MI Bridges enrollment training, and CAC curriculum completed.

Responsibilities

• Front Office Specialist for the Student Health Center; manage daily operations and patient flow.
• Conduct intake, enrollment, and referrals to MI Bridges, Sliding Fee, Healthy Michigan Plan; assist with applications.
• Coordinate with the care team to ensure coordinated, patient-centered care.
• Serve as outreach coordinator for school meetings and events; promote program awareness.
