Job summary

The Community Health Worker (CHW) at MyMichigan Health serves as a frontline liaison to connect patients with health care, community, and social services to address SDOH barriers and improve outcomes. This grant-funded, one-year position requires collaboration with ambulatory teams and Epic documentation workflows.

Primary duties include outreach, care coordination, education, and building partnerships with community resources, while adhering to reporting standards and CMS requirements.

Qualifications

• Experience in a health system, public health sector, or community benefit organization.
• Strong written and oral communication skills, including report presentation.
• Knowledge of PC applications and MS Word/Excel/PowerPoint/Project.
• Self-motivated, strong problem solver, and detail-oriented.
• Effectively communicate with community members, partners and stakeholders.
• Communicating health information with culturally appropriate terms.
• Navigate complex social/health issues.

Responsibilities

• Manage Community Health referrals for assigned geographies and conduct outreach to address SDOH needs.
• Promote wellness and health equity by providing locally relevant education and support to patients and community members.
• Maintain knowledge of SDOH best practices and CMS requirements; use Epic workflows for documentation.
