Job summary

MyMichigan Health is looking for a Community Health Worker in Alma, Wisconsin. This grant funded position coordinates outreach efforts to connect patients with healthcare resources, collaborating closely with schools and community organizations to improve health outcomes.

The ideal candidate has experience in public health and strong communication skills. Responsibilities include managing referrals, community outreach events, and ensuring accurate documentation. This position requires the CHWCoreComp certification within 6 months.

Qualifications

• Experience in a health system, public health sector, or community benefit organization required.
• Ability to effectively communicate with community members, partners and stakeholders.
• Capability to navigate complex social/health issues.

Responsibilities

• Collaborates with school personnel and community organizations for student referrals.
• Manages community health work queue and conducts patient outreach.
• Coordinates with the Community Health team for outreach events.
• Maintains knowledge of SDOH best practices and documentation workflows.

Skills

Strong written and oral communication skills

Experience in a health system or public health sector

Knowledge of MS Word, Excel, PowerPoint and Project

Self-motivated and detail-oriented

Ability to communicate health information appropriately
