Job summary

Scheurer Hospital Inc. in Harbor Beach, MI is seeking a full-time Community Health Worker to join our team. The CHW will provide outreach, care coordination and health education to support students and families at school-based clinics and Scheurer Physician Clinics.

• Two years of experience in a healthcare-related field.
• Experience with children, adolescents, families and schools for school-based clinics.
