Job summary

Crossroads Home Health and Hospice, an Impact Healthcare agency, seeks a skilled Registered Nurse to join our Vallejo-based home health team. This part-time role focuses on delivering compassionate care, assessing patient needs, and guiding families toward greater independence and quality of life.

You will visit patients in Vallejo and surrounding Solano County, complete OASIS assessments, develop individualized care plans, administer medications as ordered, and collaborate with physicians,

Qualifications

• 1 year of home health experience
• OASIS experience preferred
• CPR certification
• Driver’s license and auto insurance
• Background check capable
• Clinical judgment and critical thinking
• Communication and interpersonal skills

Responsibilities

• Conduct initial and ongoing assessments of each patient’s health status and needs
• Complete OASIS assessments accurately and on time
• Develop and implement individualized care plans based on each patient’s condition and goals
• Administer medications, treatments, and procedures as ordered by the physician
• Monitor and document each patient’s progress and response to care
• Provide education and support to patients and their families
• Collaborate with physicians, therapists, and the rest of the care team
