Job summary

Crossroads Home Health and Hospice, an Impact Healthcare agency, seeks a skilled Registered Nurse to join our Vallejo-based home health team. This part-time role focuses on delivering compassionate care, assessing patient needs, and guiding families toward greater independence and quality of life.

Qualifications

• 1 year of home health experience
• OASIS experience preferred
• CPR certification
• Driver’s license and auto insurance
• Background check capable

Responsibilities

• Conduct initial and ongoing assessments of each patient’s health status and needs
• Complete OASIS assessments accurately and on time

About the Position

We're looking for a highly skilled, compassionate Registered Nurse to join our home health team in Vallejo and the surrounding Solano County communities. This is a part time position. You'll provide skilled nursing care to patients in their homes, looking after their safety, comfort, and well-being. You'll work closely with physicians, therapists, and other healthcare professionals to build and carry out an individualized care plan for each patient, and you'll educate and support their families along the way. The goal is simple: help every patient reach their highest level of independence and quality of life.

Coverage Area: Vallejo, CA and surrounding Solano County communities. Candidates must be able to visit patients in their homes throughout the coverage area.

Compensation:$55.00 - $60.00 per hour

What You'll Do

• Conduct initial and ongoing assessments of each patient's health status and needs

What You Bring

• At least 1 year of recent experience in home health or a related field
• OASIS experience preferred
• Current CPR certification
• Valid driver's license and auto insurance
• Ability to pass a background check
• Strong clinical judgment, critical thinking, and problem solving skills
• Excellent communication and interpersonal skills with patients, families, and colleagues
• Ability to work independently and as part of a team
• Comfort with technology and electronic medical records
