Job summary

MUSC Health Florence Medical Center in Florence, SC is seeking a part-time Case Manager RN to coordinate care from admission to discharge. The ideal candidate holds a Bachelor of Nursing degree with at least one year of case management experience; licensure as a SC RN; and, if possible, BSN enrollment within two years and a case

Qualifications

• RN licensure in SC or compact state required.
• Bachelor of Nursing degree preferred; enrolled in BSN program within two years and BSN degree within four years of hire.
• One year of case management and/or utilization review experience preferred.
• Case Management certification preferred.

Responsibilities

• Coordinates the overall interdisciplinary plan of care from admission to discharge.
• Monitors care and acts as liaison between patient/family, healthcare personnel, and insurers.
• Evaluates patient needs and resources and facilitates the plan for best outcomes.
• Develops discharge plans that provide resources to meet ongoing patient needs and encourage compliance with medical advice.
• Identifies patient care issues and suggests revisions to clinical pathways to improve quality of care; may conduct utilization reviews.
