Job summary

MUSC Health invites applications for a Nurse Case Manager I in Charleston, SC. You will coordinate patient care plans, promote safe, timely discharge, and move patients smoothly through the hospital system.

Under guidance of the Case Management team, you will apply discharge planning expertise, resource utilization, and clinical case management to achieve optimal outcomes. A BSN and RN licensure with several years of experience are required, with leadership exposure preferred.

Qualifications

• Bachelor's degree in Nursing from an accredited school of nursing.
• Three years work experience as a Registered Nurse required.
• Leadership experience preferred.
• Case Management certification preferred.
• Staff hired prior to July 1, 2013, will be grandfathered in regards to education requirement.

Responsibilities

• Coordinate a care plan to achieve clinical outcomes and promote a safe and timely discharge.
• Manage the plan of care throughout the care continuum to ensure efficient patient movement through the hospital system.
• Serve as a clinical expert related to discharge planning, resource utilization, and case management.

Education

Bachelor's degree in Nursing

Three years work experience as a Registered Nurse

Case Management certification preferred

Leadership experience preferred
