Job summary

Highland Hospital in Rochester, NY seeks a part-time RN Case Manager to join the Social Work Service. The role coordinates interdisciplinary care planning to achieve timely, safe discharge and coherent post-hospital care.

Responsibilities include identifying discharge needs, closing care gaps with community resources, coordinating transitions with PCPs, and developing care plans for medically complex patients. Work is 20 hours per week, Monday–Friday, 8:00 am–4:30 pm.

Qualifications

• RN license in NYS required; BSN preferred.
• Progressive clinical experience in care management is preferred.
• Experience working with a multi-disciplinary team is required.

Responsibilities

• Coordinate interdisciplinary care planning for timely and safe discharge.
• Identify discharge barriers and connect patients with community resources.
• Communicate with in-network PCPs to ensure smooth transitions.
• Develop and implement care plans for medically complex patients.
• Refer to home care services and DME as part of discharge planning.
