Job summary

Highland Hospital in Rochester, NY is seeking a part-time Case Manager to support discharge planning, care coordination, and patient-centered management. The role works Monday through Friday, 8:00 am to 4:30 pm on site.

The ideal candidate is a licensed RN in New York with 3–5 years’ nursing experience, including community health or disease management, and experience collaborating with an interdisciplinary team to ensure safe, timely patient care.

Qualifications

• Licensed RN in New York State is required.
• 3–5 years nursing experience including community health or disease management is required.
• PRI certification preferred but not required.

Highland Hospital is seeking a part‑time Case Manager to join the team. Hours will be Monday–Friday, 8 am–4:30 pm in Rochester, NY.

Job Requirements

• Licensed RN in New York State (BSN preferred). 3–5 years professional nursing experience, including community health, care management, and/or disease management.
• Experience as a member of a multi‑disciplinary team is required. Experience in data analysis and chronic illness management preferred.
• Current and valid RN license in New York State required. PRI certification preferred but not required.

Compensation

Salary Range: $77,220.15 – $93,600 per year.
