Qualifications

• Nursing degree and state licensure required.
• Experience in hospital/home health/nursing home settings (2–4 years).
• Care management experience preferred (2–4 years).

Responsibilities

• Review medical records for admission and utilization decisions.
• Coordinate discharge plans and transitions of care.
• Collaborate with IDT for effective patient care communication.
• Refer cases to physicians/managers when criteria unmet.
• Liaise with community agencies for smooth transitions.
• Lead interdisciplinary care planning meetings.
• Document case management activities accurately and timely.
• Assist patients and families with discharge planning and post-hospital options.
