Qualifications

• Associate Degree in Nursing is required.

Responsibilities

• Conducts daily reviews of medical records to assess admission and ongoing stay.
• Collaborates with interdisciplinary teams to coordinate patient care and transitions.
• Develops and implements discharge plans and post-hospital placement.
• Refers cases to physicians or managers when criteria are not met.
• Serves as liaison with community agencies for smooth transitions.
• Facilitates interdisciplinary meetings for care planning.
• Maintains accurate documentation of case management activities.
• Provides assistance to patients, families, and physicians regarding discharge planning.
• Performs other duties as assigned.
• Maintains regular and reliable attendance.
