Job summary

Physicians Regional Medical Center in Naples, FL seeks a part-time Registered Nurse Care Manager to coordinate discharge planning, transitions of care, and case management activities for optimal patient outcomes.

You will collaborate with interdisciplinary teams, review medical records for appropriateness, and ensure compliance with standards while managing post-hospital needs and services.

Qualifications

• Associate Degree in Nursing required.
• Bachelor's Degree in Nursing preferred.
• 2-4 years of clinical nursing experience in hospital/home health/nursing home.
• 2-4 years of care management experience preferred.

Responsibilities

• Review medical records daily to assess admission necessity and utilization.
• Collaborate with IDT to coordinate patient care and avoid delays.
• Develop and implement discharge plans and post-hospital placement.
• Refer cases to physicians/managers when criteria are not met.
• Maintain timely documentation of case management activities.
• Assist patients and families with discharge planning and options.
