• Handle Plan of Care (POC) documentation for evaluations and re-evaluations.
• Track and follow up on POC communications with doctors' offices and patients.
• Monitor patient transfers and assist with queue management and care team coordination.
• Perform daily charge reconciliation and address discrepancies.
