Job summary

Hope House Treatment Center is seeking a part-time, entry-level Case Manager to support patients on their recovery journey. The role focuses on coordinating services, reducing barriers to care, and connecting patients with community resources to promote stability and sustained recovery.

The position emphasizes collaboration with the treatment team to develop discharge and aftercare plans and to empower patients to access and navigate support systems.

Qualifications

• Bachelor’s Degree in social work, human services, psychology, counseling, or related field.
• Preferred: One year of experience in case management, behavioral health, substance use treatment, social services, or related field.
• Knowledge of substance use disorders, recovery principles, and community-based support services.
• Proficiency with computers and Microsoft Office applications.

Responsibilities

• Coordinate services and discharge planning for patients.
• Assist with developing individualized discharge and aftercare plans.
• Connect patients with community resources supporting long-term recovery and independence.
• Collaborate with probation officers, social service agencies, healthcare providers, and others.
