Job summary

Hope House Treatment Center is seeking a part-time, entry-level Case Manager to support patients on their recovery journey. You will connect clients with community resources, schedule appointments, and help with discharge planning while maintaining compliant documentation.

The role emphasizes collaboration with the treatment team and advocating for patients’ access to housing, employment, and healthcare services. Bilingual Spanish is a huge plus for this position.

Qualifications

• Bachelor’s Degree (Master’s preferred) in social work, human services, psychology, counseling, or a related field.

Responsibilities

• Assist with psychosocial assessments and identify patient needs, strengths, and barriers.
• Develop individualized case management and discharge plans with patients and treatment team.
• Coordinate referrals to housing, employment, education, transportation, healthcare, and other community resources.
• Document all activities timely and accurately in the electronic health record.
• Ensure compliance with federal, state, accreditation, and organizational standards.
• Advocate for patients to address barriers to housing, employment, and healthcare access.
