Job summary

Promises Behavioral Health is seeking a passionate Full-Time Case Manager with weekend availability to join our team. The role involves coordinating comprehensive services during residency and planning for continuing care after discharge.

Responsibilities include screenings, service planning, and delivering psychoeducational groups, while maintaining accurate documentation and coordinating housing and care resources.

Qualifications

• Bachelor's degree preferred or equivalent experience.
• Knowledge of case management and related services.
• Experience with substance abuse, mental health, and housing supports.

Responsibilities

• Screen, intake, orientation, and assessment of patients.
• Develop and refine individualized service plans and referrals.
• Conduct at least three case management sessions per patient weekly.
• Document in the client medical records and follow up after discharge.
• Coordinate resources for continuing care and safe housing.
• Deliver psychoeducational groups and motivational enhancement.
• Attend meetings and training to ensure quality of care.
