Job summary

Horizon House of Illinois Valley, Inc. is seeking a part-time (20 hours per week) QUALIFIED INTELLECTUAL DISABILITIES PROFESSIONAL (QIDP) specializing in SELF-DIRECTION ASSISTANCE (SDA).

The Team Leader's main functions include assisting persons served to identify outcomes, design person-centered service plans, coordinate support teams, monitor plan effectiveness, and advocate for changes based on personal outcomes.

Qualifications

• EDUCATION: Bachelor's Degree in Human Services Field which meets federal regulatory requirements for a Qualified Intellectual Disabilities Professional (QIDP)
• EXPERIENCE: One (1) year Direct experience which meets regulatory requirements for a Qualified Intellectual Disabilities Professional (QIDP).

Responsibilities

• Lead and advocate for persons served to identify and review their outcomes
• Coordinate activities of support teams and monitor plan effectiveness
• Ensure plan delivery with fidelity across service settings
• Complete documentation and maintain data related to implementation strategies and monitoring
• Assist in maintaining compliance with applicable laws and agency policy
• Participate on agency committees as assigned
