Job summary

HealthPro Heritage in Sunnyvale, CA is seeking a part-time Physical Therapist Assistant for outpatient therapy services. Join a 90-bed community spanning Assisted Living, Independent Living and Memory Care, delivering outpatient care to residents.

The position is up to 25 hours weekly with a pay range of $40-$44 per hour and PRN availability, offering flexible scheduling and a supportive, activity-focused team.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
• Proficiency with diagnostic tools, therapy equipment, relevant software, and electronic health record systems.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists. Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
