Job summary

CareMed seeks a Wound Care Nurse Practitioner to deliver comprehensive wound care to residents in long-term care facilities. You will prescribe medications per West Virginia law, coordinate treatment plans, and work with the facility’s interdisciplinary team to maintain optimal patient outcomes.

Ideal candidates hold an unrestricted WV NP license, board certification, and have experience with wound care including VAC/advanced dressings, with a focus on geriatric and chronic wounds.

Qualifications

• Current, unrestricted NP license in West Virginia.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family or geriatric care.
• Clinical wound-care background including chronic, infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, or mixed wounds.
• Ability to stage pressure injuries per NPIAP.
• Familiar with wound care products and advanced dressings.
• Experience with wound VACs and other DME.
• Strong preference for wound certification or willingness to obtain it.
• Experience in long-term care and EMR usage.
• Ability to work within an interdisciplinary team.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.

• Prescribe appropriate medications, including controlled substances in accordance with WV Law.

• Order, review, and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS:

• Current, unrestricted NP license in the state of West Virginia.

• Board Certification.

• DEA licensure (Optional)

• Clinical background in adult, family, or geriatric specialties.

• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

• Must be comfortable staging pressure injuries according to the NPIAP

• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

• Strong preference to having wound certification or be open to getting certified

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to work within an interdisciplinary care team to coordinate patient care
